Nurse Practitioner / Physician Assistant

Location: Seattle, WA
Compensation: $135,000 – $160,000 per year
Job Type: Hybrid (Field + Remote/Admin)

Summary

We are seeking a dedicated Nurse Practitioner or Physician Assistant to join the team. In this role, you will provide comprehensive medical care to senior patients within local assisted living facilities and assist in creating comprehensive clinical plans for patients.

Your role would involve seeing roughly 25–40 patients per week in person (3–4 days per week) at assisted living facilities, with approximately 1–2 days per week involved in care management work and telemedicine visits (virtual) with approximately 6 billable hours per day expected on virtual days. When you are out in the field, you will typically be at only one facility each time, with care managers supporting you every step of the way in managing your patient.

You would be supported by a lead internal medicine physician/cardiologist who can provide medical expertise, in addition to a team of highly skilled care managers to ensure the plan is implemented.

About 25% of time is engaged in care management and virtual work, with significant scheduling flexibility and the ability to choose your own shifts. Patient volume would be about 12–14 patients per shift max (average 10–12), allowing you to spend additional time with patients. You would be able to be a part of a small, cohesive startup group that is building the future of medical care for patients in their home.

Your role would be uniquely supported by care managers who know their patients well, reducing busy work. This means that most follow-ups including labs, ordering studies, and referrals would be completed by care managers, decreasing the administrative burden. You would have a small panel of patients and be able to develop strong relationships with the patients you work with. Scheduling is flexible and designed to accommodate employee goals.

Responsibilities

  • Conduct thorough assessments of patients' health status and develop individualized care plans
  • Ability to drive to facilities to conduct visits within a 30-mile radius of Seattle, WA
  • Work collaboratively with a team of care managers and physician dedicated to comprehensively assisting vulnerable patients
  • Provide education and support to patients and their families regarding health management
  • Collaborate with interdisciplinary teams including assisted living facilities, psychologists, and clinical teams to coordinate comprehensive patient care
  • Monitor patients' progress and adjust treatment plans accordingly
  • Ensure compliance with healthcare regulations and standards

Requirements

  • Valid Nurse Practitioner license in Washington or Physician Assistant License
  • Clean driving record
  • Experience in senior care or home healthcare preferred (bonus for assisted living experience)
  • Strong knowledge of care management and primary care
  • Excellent communication skills for effective patient interaction
  • Ability to work independently and as part of a team
